Grounds for data collection
Processing of your personal information which may include names, addresses, e-mail addresses, phone numbers, billing information (including business account information for billing, payment or reimbursement/refunds) is necessary for the performance of our contractual obligations towards you and providing you with our services, to protect our legitimate interests and for compliance with legal and financial regulatory obligations to which we are subject. "Users" or "You" may include but is not limited to a business entity or contracting agent or entity, staff or employees registering or contracting for services as individuals or employees of a booking agent or business, company, or vendors in which we or the user operates, trains, or contracts with. Registration for trainings and services may require collection of gender, birthdate, or other personably identifying characteristics as required for registration within or on State of Wisconsin databases. These may include but may not be limited to Caregiver registries. The Wisconsin Community-Based Care and Treatment Training Registry keeps track of those individuals who meet criteria of state-mandated trainings for community settings and required information will need to be reported to the licensing/accrediting/monitoring body. This information can not be declined to be provided nor falsified.
When you use the Site, you consent to the collection, storage, use, disclosure and other uses of your Personal Information as described in this Privacy Policy.
We encourage our Users to carefully read the Privacy Policy and use it to make informed decisions.
What information we collect?
We collect two types of data and information from Users.
The first type of information is un-identified and non-identifiable information pertaining to a User(s), which may be made available or gathered via your use of the Site (“ Non-personal Information
”). We are not aware of the identity of a User from which the Non-personal Information was collected. Non-personal Information which is being collected may include your aggregated usage information and technical information transmitted by your device, including certain software and hardware information (e.g. the type of browser and operating system your device uses, language preference, access time, etc.) in order to enhance the functionality of our Site. We may also collect information on your activity on the Site (e.g. pages viewed, online browsing, clicks, actions, etc.).
The second type of information Personal Information
which is individually identifiable information, namely information that identifies an individual or may with reasonable effort identify an individual. Such information includes:
- Device Information: We collect Personal Information from your device. Such information includes geolocation data, IP address, unique identifiers (e.g. MAC address and UUID) and other information which relates to your activity through the Site
- Any information required for booking, contracting, billing or payment for services will also be collected as identified above.
- Registration information: When you register to our Site you will be asked to provide us certain details such as: full name; e-mail or physical address, and other information.
How do we receive information about you?
We receive your Personal Information from various sources:
- When you voluntarily provide us your personal details in order to register on our Site;
- When you use or access our Site in connection with your use of our services;
- From third party providers, services and public registers (for example, traffic analytics vendors).
What information we collect?
We do not rent, sell, or share Users’ information with third parties except as described in this Privacy Policy.
We may use the information for the following:
- To register you accordingly to required oversight agencies or bodies within the State of Wisconsin Department of Health Services.
- Communicating with you – sending you notices regarding our services, providing you with technical information and responding to any customer service issue you may have;
- To communicate with you and to keep you informed of our latest updates and services;
- To serve you advertisements when you use our Site (see more under "Advertisements");
- To market our websites and products (see more under "Marketing");
- Conducting statistical and analytical purposes, intended to improve the Site.
- Billing, payments, refunds and collections as applicable to services rendered. We may store billing and payment information for future credits, refunds, debits, or payments.
In addition to the different uses listed above, we may transfer or disclose Personal Information to our subsidiaries, affiliated companies and subcontractors.
In addition to the purposes listed in this Privacy Policy, we may share Personal Information with our trusted third party providers, who may be located in different jurisdictions across the world, for any of the following purposes:
- Hosting and operating our Site;
- Providing you with our services, including providing a personalized display of our Site;
- Storing and processing such information on our behalf;
- Serving you with advertisements and assist us in evaluating the success of our advertising campaigns and help us retarget any of our users;
- Providing you with marketing offers and promotional materials related to our Site and services;

Retention
We will retain your personal information for as long as necessary to provide our services, and as necessary to comply with our legal obligations, resolve disputes, and enforce our policies. Retention periods will be determined taking into account the type of information that is collected and the purpose for which it is collected, bearing in mind the requirements applicable to the situation and the need to destroy outdated, unused information at the earliest reasonable time. Under applicable regulations, we will keep records containing client personal data, account opening documents, communications and anything else as required by applicable laws and regulations.
We may rectify, replenish or remove incomplete or inaccurate information, at any time and at our own discretion.
Cookies
We and our trusted partners use cookies and other technologies in our related services, including when you visit our Site or access our services.
A "cookie" is a small piece of information that a website assign to your device while you are viewing a website. Cookies are very helpful and can be used for various different purposes. We also use a tool called “Google Analytics” to collect information about your use of the Site. Google Analytics collects information such as how often users access the Site, what pages they visit when they do so, etc. We use the information we get from Google Analytics only to improve our Site and services. Google Analytics collects the IP address assigned to you on the date you visit sites, rather than your name or other identifying information. We do not combine the information collected through the use of Google Analytics with personally identifiable information. Google’s ability to use and share information collected by Google Analytics about your visits to this Site is restricted by the Google Analytics Terms of Use
and the Google Privacy Policy.
Third party collection of information
Our policy only addresses the use and disclosure of information we collect from you. To the extent you disclose your information to other parties or sites throughout the internet, different rules may apply to their use or disclosure of the information you disclose to them. Accordingly, we encourage you to read the terms and conditions and privacy policy of each third party that you choose to disclose information to.
This Privacy Policy does not apply to the practices of companies that we do not own or control, or to individuals whom we do not employ or manage, including any of the third parties which we may disclose information as set forth in this Privacy Policy.
How do we safeguard your information?
We take great care in implementing and maintaining the security of our Site and your information via SSL and HTTPS technology and password enhanced email . Although we take reasonable steps to safeguard information, we cannot be responsible for the acts of those who gain unauthorized access or abuse our Site, and we make no warranty, express, implied or otherwise, that we will prevent such access.
Advertisements
We may use a third-party advertising technology to serve advertisements when you access the Site. This technology uses your information with regards to your use of the Services to serve advertisements to you (e.g., by placing third-party cookies on your web browser).

Marketing
We may use your Personal Information, such as your name, email address, telephone number, etc. ourselves or by using our third party subcontractors for the purpose of providing you with promotional materials, concerning our services, which we believe may interest you. We, however, will not reach out to a customer who has not first expressed interest in our services or been a prior client, user, or contractor. We my also use your contact information to notify any purchasing party of changes to the privacy policy.
Out of respect to your right to privacy we provide you within such marketing materials with means to decline receiving further marketing offers from us. If you unsubscribe we will remove your email address or telephone number from our marketing distribution lists.
Please note that even if you have unsubscribed from receiving marketing emails from us, we may send you other types of important e-mail communications without offering you the opportunity to opt out of receiving them. These may include customer service announcements or administrative notices.
Corporate transaction
We may share information in the event of a corporate transaction (e.g. sale of a substantial part of our business, merger, consolidation or asset sale). In the event of the above, the transferee or acquiring company will assume the rights and obligations as described in this Privacy Policy.
Minors
We understand the importance of protecting children’s privacy, especially in an online environment. The Site is not designed for or directed at children. Under no circumstances shall we allow use of our services by minors without prior consent or authorization by a parent or legal guardian. We do not knowingly collect Personal Information from minors. If a parent or guardian becomes aware that his or her child has provided us with Personal Information without their consent, he or she should contact us at bducane@carewiseconsulting.com. No minor shall be allowed to enroll, train, attend, or otherwise pay for or finance any contract or training/educational session without a legal parent or guardians express written consent as the primary obligator. No minor under the age of 16 will be allowed to attend, enroll, or finance any training opportunities or services provided by Care Wise Consulting LLC.
Updates or amendments to this Privacy Policy
We reserve the right to periodically amend or revise the Privacy Policy; material changes will be effective immediately upon the display of the revised Privacy policy. The last revision will be reflected in the "Last modified" section. Your continued use of the Platform, following the notification of such amendments on our website, constitutes your acknowledgment and consent of such amendments to the Privacy Policy and your agreement to be bound by the terms of such amendments.

Terms and Conditions of Service
All class fees paid are non refundable. Failure for a enrollee to show or attend a scheduled class or training without prior notice is at the expense of the purchaser to recover all cost from the absent enrollee. Care Wise Consulting is a non-recoverable source for payments issued if an enrollee fails to show as scheduled according to the terms here in. No student shall be allowed to participate in any class if more than fifteen minutes tardy from the scheduled start time. We request all student arrive ten minutes prior to class start time for roll call and sign in. Group discounts apply to a single entity bookings and is not applicable to total sum of enrollees in any given educational training session.
Listed cost for educational services and products rendered include fees for registry placement and e-learning materials. Care Wise Consulting makes every effort to place students in full compliance status on the registry within 48 hours. In rare occasions with extenuating circumstances it may take up to the 10 days allowed by law.
Extenuating provable circumstances may apply and allow student to enter another class if space is available. The class may not be at the original booked site and may entail travel to a Host Facility or company sponsored site. Extenuating circumstances include death of an immediate family member (Grand parent, parent, spouse, sibling, or child) of an attendee within 96 hours of the expected class start, unexpected hospitalization or motor vehicle accident involving injury rendering them unable (not unwilling) to attend, natural disaster, or acts of terrorism. Proof may be required in the form of obituary, police report, or medical documentation and must contain the enrollees direct name. Proof must be submitted within ten business days (excluding federally sanctioned holidays) to billing@carewiseconsulting.com .
Ten business days starts on the day the class was set to meet initially or start if it is more than a 1 day enrollment.
Full cancellation with refund will apply only to cancellations made 24 hours or more in advance of a scheduled educational training start time. Cancellations less than 24 hours will be rebookable as credit forward for the same enrollment that was late cancelled. Cancellations less than 4 hours will be non refundable unless the Extenuating provable circumstances apply and may be subject to forfeiture based on class size.
Booking two enrollees and cancelling one of those two booked students may cause the price to be charged at a single person rate as though it was an individual attendee training (a one person one instructor training session) unless other attendees meet minimum requirements set forth for each class. This price is calculated as: two enrollee fee divided by two plus $150 for Medication Aide. This price is calculated as: two enrollee fee divided by two plus $100 for all other DHS 83.20 training sessions. This calculation may apply only in the circumstance that a two student class is not met due to a less than 4 hour cancellation causing a two enrollee booking to become a single.
Credits or debits on fees for services may be applied to negative balances or credited to future enrollments. Direct refunds must be requested in writing via email from the corporate contact email address for facilities enrollees and may take up to 30 days to process. Direct refund request should be emailed to: billing@carewiseconsulting.com
Failure of student to attend may cause cost to be adjusted accordingly to purchasers as laid out in the fee schedule page.
Proof of a student death occurring prior to class start will allow full refund for the classes enrolled without an increase in the fee structure being applied. It may also result in monies owed to the booking agent/company/purchaser, however will be credited to any account with a negative balance before any refunds are issued.
Upon proof of an unexpected attendees death the purchasing agent must notify Care Wise Consulting and if a positive or zero balance is contained within the account for services performed, a full refund, or remaining balance refund will be issued. Accounts with an initial negative balance will have the difference refunded if applicable. Notice of unexpected attendee death must be in writing with proof (obituary or death notice) via email within 14 days of the enrollees death from the corporate contact email address and may take up to 30 days to process. Direct refund request due to death should be emailed to: billing@carewiseconsulting.com .
Any representative enrolling or purchasing services for an enrollee on behalf of a company acknowledges that the individual has had their background check completed by the enrolling or purchasing agent or company in accordance to Wisconsin State law. They further acknowledge the enrollee is eligible to be employed as a caregiver in the State of Wisconsin, and agree to make these available to the company or any of its employees upon request prior to or at the start of any educational services provided.
Care Wise Consulting LLC will not be held liable for fees of services rendered for individuals, facilities, entities or purchasing agents who fail to verify an enrollees eligibility to perform as a caregiver in the State of Wisconsin prior to paying any associated fees or cost for educational and training services.
Purchasers of services will be required and agree to provide proof of completed care giver background checks for enrollees attending at a Host Facility (a facility not of their own employ or owed by or acting as a subsidiary of that employing company). Failure to provide the completed caregiver back ground check a minimum of two hours prior to an enrollees scheduled class start at a Host Facility will result in disqualification of an enrollee to participate in an enrolled class and forfeiture of fees paid for services unless other arrangements have been made prior to the enrollees scheduled start time (e.g. the enrollee will be bringing a paper copy with them to the class).
Care giver background checks may be emailed to any Care Wise Consulting email address or faxed to the main fax number listed on the contact page. The time stamp on the receivers fax or email shall serve as valid proof of time of receipt by Care Wise Consulting; certain exceptions may apply based on instructor ability to receive such documents after the cut off time frame.
The contracting, purchasing, or enrolling party may be held legally liable for any damages caused by misleading statements or falsified documents submitted in reference to the completion of the Wisconsin Caregiver background check and its applicable laws.
Care Wise Consulting LLC does not assume liability for students personal actions while attending any of the companies educational offerings nor does Care Wise Consulting guarantee that a student will successfully complete any or all enrolled trainings. Care Wise does not educate on professionalism and boundaries and may dismiss an enrollee for inappropriate behavior including but not limited to sleeping during enrolled trainings, inappropriate or frequent unrelated disruptions, inappropriate demeanor or actions toward other students, drug or illicit substance use, or acute intoxication, and harassment based on any federally protected status. Refunds will not be offered if a student is dismissed for inappropriate behavior, conduct, or professionalism and becomes responsibility of the purchasing agent to seek recovery from the enrollee. Dismissed students may be ineligible to attend further enrollments with Care Wise Consulting. A full course retake fees will apply to any courses not successfully completed.
All disputes arising from the Terms and Conditions of Service should first be emailed to the attention of Bryan Ducane: bducane@carewiseconsulting.com for review and resolution. By purchasing any products or services contained within the Care Wise Consulting LLC's website, the contracting or purchasing party agrees to first contact the above person at the above e-mail address to attempt resolution. If resolution can not be amicably reached to the satisfaction of both parties, the parties agree to independent arbitration versus a court of law and waive such rights for action in a court of law under local, county, municipal, district, state, tribal, and federal jurisdiction. The Terms and Conditions of Service are applicable to any individual enrollee, contracted agent, contracted facility, or facility representative who may be responsible for enrolling, purchasing, and assuring payment for services rendered for a facility or those acting in that capacity. Legal fees including, but not limited to filing fees, lawyer fees, lost time, and loss of goods and services may also become responsibility of a delinquent party who has failed to meet responsibilities or obligations for services outlined in these terms and conditions or as negotiated in a signed contract. Any individual who enrolls in services through any online, phone, email, or facsimile means is also subject to the terms and conditions of service for payment received or held for services that were to be rendered.
By enrolling in any of the education or training offerings contained within Care Wise Consulting LLC's website, any purchaser providing payment for or acknowledgment to provide payment for services by enrolling attendees or oneself in any of Care Wise Consulting LLC's educational trainings or consultant services, acknowledges acceptance of these Terms and Conditions of Services.
Host Facility Terms of Service
Discount of 10% applies for all billed or paid charges of enrollees attending as individuals or attendees from an outside entity utilizing Care Wise Consulting LLC's educational training products. E.g. : A host facility opts for a 3 student Med Aide class and all enrollees show. The facility is charged $630.00. An outside entity books a class for 2 enrollees at $450.00 and 1 individual enrollee attends at $375.00 at your host facility. The ten percent discount rewarded is of the total cost of hosted enrollees from outside entities. $450. plus $375. equals $825.00. Ten percent of total hosted enrollees equates to $82.50 discount. Host Facility savings are advanced prior to billing (monthly) for post paid accounts and credited forward for prepaid accounts (with next purchase). Care Wise Consulting LLC does not guarantee outside enrollees or discounts if no outside enrollees attend. Host Facility discounts apply only when enrollees purchase and attend educational services.
Care Wise Consulting LLC requires a Host Facility have ample space for equipment , supplies, and seating with table or counter top space for up to ten students and up to two instructors. Climate controlled room adequate for the season with sufficient lighting and electrical outlets for instructor needs (2 minimum). Table or counter top space not less than 2 feet by 4 feet at seated or waist height for instruction and demonstration. *Internet speeds should be available at speeds of no less than 10mbps downlink by 3mbps uplink and available via wireless or ethernet connection. *Wall space for projection that is at least 36 by 24 inches (43 inches diagonal) or connectable accessible (not password protected) TV or Monitor for attaching or mirroring materials and videos via HDMI cable. A bathroom with hand washing facilities must also be available unlocked or locked with a key or passcode available.
Care Wise Consulting LLC assumes liability for students activities only while performing under the direct supervision of our licensed instructors during the scheduled training hours attended relating to the materials presented. This will apply to the 2 hour revision period for the Certified Med Aid Class if it is needed for enrollee development.
Care Wise Consulting LLC can not and will not assume liability for injury to an enrollee or attendee caused by a Host Facilities visitors, staff, maintenance, facilities upkeep, or equipment, or lack thereof, whether intentional or unintentional. This includes occurrences within the interior and exterior of dwellings and buildings of a Host Facility, building entrances and exits, hallways, corridors, and rooms contained within those buildings of the Host Facility, parking areas (whether dedicated as such or not), lawns and landscaping, sidewalks, gutters, curbing, over hangs, garages or other protective parking structures, driveways, and lots-paved or unpaved, and any easements owned, leased or rented by a host facility.
Host Facility Terms of Service apply only to contracted entities receiving the discount listed above.
If applicable: Terms and Conditions of Service may vary and be individualized to the contracted Educational and Consultant services. See your contract for further information and details.
All contracts are legally binding once negotiated and signed by both parties. Contracts are bound to the legal terms and conditions forestated within these Terms and Conditions of Service unless otherwise negotiated and termed into the individual contract.

Care Wise Consulting LLC's Terms of Service and Privacy Policy may change at any time with or without notice. Care Wise Consulting LLC will make all possible attempts to notify all contracting vendors. agents, users, facilitates, or enrollees of any changes whom have a valid e-mail address on file. Care Wise Consulting LLC can not guarantee updates will be provided in the event of non-usage of our services of greater than 30 days or in the event of bounce-back emails. Care Wise will not accept responsibility for failure to receive these updates due to emails being delivered to any spam, trash, junk, or any other unmonitored email address or inbox.
